XIX. CONSIDERATION FOR SERVICES
The amounts, which are mentioned below are to be paid by the Couple of the Intended Parents to the Surrogate Mother in consideration of all services to be rendered, time to be spent, pain along with suffering, personal discomfort, child support, living expenses and other personal inconveniences and costs, in performance of all the terms and obligations set forth in this Surrogacy Agreement. All payments, which are made to the Surrogate Mother, are to be made by the Couple of the Intended Parents, and any of these payments are neither to be contingent upon surrender of the Child / Children to the Couple of the Intended Parents, nor upon the Surrogate Mother’s waiver of rights and / or responsibilities, which are pursuant to this Surrogacy Agreement. All these payments are to be made in the amount and at the definite times to the definite persons or entities, as it is indicated below:

A. The costs and fees of medical service providers for the initial medical testing, which are necessary to determine the suitability of a Surrogate Mother as a traditional surrogate.
B.  The costs and fees of medical service providers for the Artificial Insemination procedure as well as all related pre – insemination medical expenses.
C. All medical, hospital, pharmacy as well as laboratory expenses, which are associated with any testing, conducted at the request of the Surrogate Mother’s gynecologist, obstetrician or midwife, which are not payable under the Surrogate Mother’s health insurance.
D. All medical, hospital, pharmacy, laboratory, as well as therapy expenses, which are incurred by the Surrogate Mother either during or resulting from any complication, arising from Artificial Insemination procedure, which are not payable under the Surrogate Mother’s health insurance.
E. The total sum of money, paid to the Surrogate Mother is ___ $; it is considered to be subject of provisions of this subparagraph, subparagraphs Q and R, described below, as well a paragraphs III and IV hereof, payable to the Surrogate Mother as full compensation for child support, living expenses, personal discomfort to the Surrogate Mother, which occurred in the result of this Surrogacy Agreement. The Surrogate Mother will receive this sum in payment of ___ $ per month. The first payment will be made within five (5) first days after the performance of confirmation of pregnancy via either blood or urine test. This confirmation is to be performed in a medical setting by either a doctor or his staff, and it is to continue on the first of every consecutive month thereafter during the whole period of pregnancy of the Surrogate Mother, up until a total of ___$ has been awarder to the Surrogate Mother.
F. The costs and fees of medical service providers for paternity testing, which is pursuant to Paragraph III, subparagraph B of this Surrogacy Agreement, in case if they are requested by the Couple of the Intended Parents.
G. The attorney fees of the Surrogate Mother for independent legal advice as for the meaning and consequence of this Surrogacy Agreement, however, not for any legal advice or representation, which has anything to do either with breaching or with enforcement of this Surrogacy Agreement. The Surrogate Mother and her Husband, if there is any, are to represent that they have retained an attorney of their own choice for the purposes for negotiating and reviewing this Surrogacy Agreement, and that this attorney has advised them as to the terms and effects of this Surrogacy Agreement. It is also possible that a court may award the fees of an attorney to any prevailing Participant in any action, which is related to this Surrogacy Contract
H. All court costs, copying fees as well as filing fees for a judicial paternity and maternity declaration.
I. The attorney’s fees of the Surrogate Mother for legal representation, which have the relation to obtaining a Judgment of the Maternity and / or Paternity, and / or adoption, in case if it is required.
J. All travel expenses of the Surrogate Mother, along with her lost wages, telephone calls and miscellaneous expenses will be paid by the Couple of the Intended Parents in a timely manner, which will not last longer than one month beginning from the submission date. The Surrogate Mother does acknowledge that she is responsible for the itemized statement of expenses to the Couple of the Intended Parents submitting in order to receive reimbursement from them. It is possible that the Surrogate Mother will subtract expenses from an advance for various upon agreed expenses.
K. The real fee or cost charged by a mental health care professional for the psychological testing conducting and / or counseling for the Surrogate Mother before the Artificial Insemination procedure, as well as during the pregnancy term and for a period of no longer than three (3) months after the delivery occurs, in case if it is advised by the mental health care professional, either through support group meeting or individual counseling procedure.
