Guidelines for Managing Surrogacy

There are protocols and guidelines for managing surrogacy in most infertility clinics and surrogacy agencies. These kinds of clinics and agencies accept infertile couple for treatment if both the genetic and host couples fulfill their criteria.
Guidelines for Managing Surrogacy
To the most necessary and widespread protocols and guidelines for managing surrogacy belong the following ones:

Consultation
The consultation is provided with a medical doctor in order for him to see whether the couple of the potential intended parents is suitable for the further treatment from the medical point of vies.

Assessment of Genetic Couples
For the gestational (In Vitro Fertilization) surrogacy, usually the ages of commissioning genetic couples of potential intended parents is to be under 35 years for women and under 55 years for men, however, it depends on the state or on the country. Considering traditional surrogacy, there are no restrictions in age for men.
The whole review of the medical history of both future intended parents as well as a complete physical examination is usually performed. The future intended father usually undergoes a semen analysis.

Semen analysis
The semen analysis test is truly considered to be one of the most important male infertility tests, and, moreover, it is the first step in the medical evaluation of male infertility process. This kind of test is generally performed somewhere after 3 – 5 days of having a sexual contact. As a rule a man receive detailed written instruction on how should his semen be collected. Usually the semen sample is collected in the result of masturbation in a special sterile pot. However, it becomes very stressful for some men to produce their semen sample by masturbating. These men should better ask for help, and their wives are the possible “assistants” in this process. There are also some other methods available, such as mechanical vibrators, Viagra tablet, which can possibly lead some men to an erection. However, in case if all these methods fail, it is also possible to collect the semen sample into a special condom, during the regular sexual intercourse. After that the volume of the ejaculate is being measured and its liquefaction is being estimated. The following step in this analysis is to investigate a drop of semen under a microscope; in order to count the number of sperm, to estimate how they can move around and it is also possible to estimate the proportion of normal sperm.

Normal values of semen variables
Volume:  not less than 2.
Liquefaction: normal (right after the ejaculation took place, the normal semen is to contain multiple jelly like particles, which should become more or less completely liquid after about half an hour).
Count: not less than 20 million sperm per ml.
Morphology: not less than 30% of the sperm are to be of a normal shape.
Motility: not less than 50% of the sperm are to be swimming and moving forward.
Vitality: not less than 75% of the sperm are to be alive.
Cells: there should be no more than 1 million of white cell count per ml. In case if there are more white cells, this may be an indication of some kind of infection.

However, it should be pointed out that only a normal semen analysis is considered to be reassuring. In case if the test is abnormal, it is necessary to repeat it two to three times over two to three months in order to confirm if the abnormality is of a persistent manner or not. Thus, it is also possible to have an abnormal test for fertile men; that is why any single semen analysis test is not significant. Most doctors also request two or there samples of semen to be examined over an interval of about two weeks, because of the possible natural variation in the quality as well as in the quantity of sperm. Therefore, it is not possible to make any kind of conclusion that any man with low sperm count is infertile, because there are a lot of men with low or moderately low sperm counts, however, they have no fertility problems. Moreover, no one can say with certainty that any man, who has normal sperm count is fertile because his sperm may be possibly lacking fertilizing capacity, or it may also be of a low quality.
